YOU MUST FILL IN THE REQUIRED DATE SECTION
If signing up for Weekly – Type in the date you are paying for.
If signing up Monthly or for TWO WEEKS, please list the month and or the 2 weekly dates.
If Seasonal – please type in “Seasonal” in the required Date Section
